We wish to make this test more robust by using medium unmodified HTF without the addition of albumin, as recommended in Oswaldien et al (Human Reproduction July 2000). | We've recently begun looking at early cleavage as a possible indicator of embryo quality. My question is about what the expected rate of early cleavage should be. We see about 30% overall but it varies according to medium. We assess at 26 hours post insemination and see the best rates (>30%) in embryos that were inseminated in IVF medium (Vitrolife), a slightly lower rate with ECM (Irvine Scientific) and an alarmingly low rate (<20%) in G1 (Vitrolife). | Recently in a case of ICSI, we had 17 eggs, only 11 MII, 2 leaked cytoplasm during denudation. 3 degenerated just after ICSI, leaking of cytoplasm was observed. 5 fertilised and only 2 cleaved with lots of fragmentation. 5 MI were also injected and they degerated too. Patient took high dose of lupron in early stage of stimulation by mistake, whish was the only abnormal thing, otherwise everything was as routine with other patients. What could be the possible reason for fragile oolema and Zona? D.. |

| Has anyone had a case like this? We have a patient who has had 1 IVF cycle using her own eggs, which generated no normal embryos, only polypronucleate fertilisation. She then had ICSI and had normal fertilisation but at a low rate (1/9) and no pregnancy. She has just had an IVF cycle using donated oocytes from an unrelated donor and had 1 normal embryo, but 7 with multiple pronuclei. Is this a sperm problem and if so, what is the mechanism? |
